JCUMSA Store Ticket Transfer Application

Please fill out the following form if you are requesting a ticket ownership transfer for a purchased item off the JCUMSA online store from the JCUMSA Treasury. This form is to be filled out by the intended new ticket holder. All sections must be filled out correctly for your application to be processed successfully. Note that all applications are treated on a case-by-case basis and may not always be approved. If the original ticket holder bought multiple tickets with their order and is selling multiple tickets, all new ticket holders must submit this form individually.
