JCUMSA Reimbursement Form

Please fill out the following form to request a reimbursement from the JCUMSA Treasury. All sections must be filled out correctly for your application to be processed successfully. Note that all applications are treated on a case-by-case basis and may not always be approved. Submissions from this form are compiled into a secure database that may undergo investigation by a third party auditor. Any submission that we suspect to be fraudulent in nature will be reported to the appropriate authorities.
